Community Health and Wellness
Atlantic General Hospital's 2020 Vision is framed around the triple
aim of the Affordable Care Act: improve the health of the population,
enhance the patient care experience, and reduce the cost of care.
We are in the process of expanding existing community resources and creating
new ones to achieve this triple aim. This section of the website is devoted
to those services and to you, a valued member of our community.
Care Coordination
Atlantic General Hospital and Health System provides care coordination
for primary care patients through the
Patient-Centered Medical Home (PCMH) and for discharged hospital patients through the Transitions in Care Program.
Our care coordinators and case managers help program participants schedule
physician follow up visits and set up appointments for tests and procedures,
while ensuring that all healthcare providers involved in an individual's
care are communicating appropriately to achieve the best possible health
for the patient.
AGH Healthy Happenings Community Education Program
It is the mission of AGH Healthy Happenings is to assess community health
educational needs and major health concerns in our community, to create
and plan an educational program that appropriately addresses these, and
to evaluate their effectiveness in reaching our institutional mission
at Atlantic General Hospital to improve individual and community health.
The program provides a number of free services to the area, including free
monthly blood pressure screenings, health fairs, and
chronic disease management education.

The program also offers a
speakers bureau, which facilitates guest speakers for events when a health care expert
is needed.

AGH Healthy Happenings Advisory Committee
The AGH Healthy Happenings Advisory Committee consists of more than 20
members and includes health specialists and coalition partners. The committee
meets monthly to address community needs and interests, plan and implement
programs, and evaluate responses and results.
Faith-Based Partnership
Atlantic General Hospital and seven area houses of worship joined forces
in 2010 to create a
Faith-Based Partnership that offers free health management services to their congregations to
help them adopt healthier lifestyles to improve their overall health and
wellbeing.
