Faith-Based Partnership

The Mission
Faith Partnership is a cooperative effort for local Worship Centers, with
Atlantic General Hospital and Health System, to increase health awareness,
education, and healthy living incentives for our community members.
The Beginning
Atlantic General Hospital and seven area churches joined forces in 2010
to offer free health management services to the community to help them
adopt healthier lifestyles and change the healthcare paradigm from “healing”
to “preventing.” These efforts take place in the partnering
churches where parishioners are better able to access the resources. Educational
sessions on pursuing and maintaining a healthy lifestyle.
The Details
Faith Partnership consists of community Worship Centers that have appointed
a Health Ambassador(s) to represent each center at the monthly meetings.
The Faith Partnership Program
meets on the first Tuesday of each month from 10:00 to 11:30 a.m. at the
Atlantic Health Center. The meetings focus on health and wellness topics and often a guest speaker
with expertise in their health topic or community resource attends the
meeting to provide education to the group. The Health Ambassadors take
the information from the meeting back to their Worship Center to share
with their congregation.
Want to become a Health Ambassador?
To be a Health Ambassador, you must be a member of a local worship center,
have a positive attitude and great people skills, be committed to your
faith, and attend the monthly Faith Partnership meetings. People who might
be a good fit to be a Health Ambassador are:
- Parents with a good network of other parents
- People with leadership or influence within a church, such as a small group
leader, clergy, elder, deacon, or board
- Leader of personal development or practical living ministries
- Stevens ministers, communion ministers, ministers to the sick, pastoral
ministers or chaplains
- Personal trainers or life coaches
- Nurses
- Physicians
- Nutritionists or dietitians
- Integrative health practitioners
- Therapists
- Teachers
- Social Workers/counselors
- Community health Workers
Resources
All community resources from services ranging from transportation services
to home health agencies and more are maintained electronically through the
Network of Care website.
